The above error comes up, when the AP transmits a key to a station; it expects a response back within certain duration (1000ms as per the standard). If the station does not respond, the AP increments the counter and retransmits a new key. If the AP receives a response to first key message just after the retransmission of the new key, a mismatch occurs in the counter.

Another reason, could be that key received by the AP but processed very late (after a new key retired from the client) resulting replay counter mismatch.
