did you change the Quik mode selector ip address after setup ? MR4 patch
7 has a bug we found with that but deleteing the phase one and two and
recreating it seemed to fix it.
your user DN is off it should look more like set dn "
OU=Users,DC=Domain,DC=local" the DN need to be set to the OU the users
are in and the LDAP account need read access to that OU
