I had that same new problem intermitent with a few of my customers running 6.0.14 or 5.6.11. The Fortigate seem to keep in memory the IP address of the previous connections from that same user, doesnt clear his cache.

I Had the same problem in Proxy-base Inspection Mode, ( 5.4.4 ). I updated AV definition from 52.00000 to 52.00003 and it fixed the problem .

Good news, in release 5.4.3 the Create New button is back in the Web Rating Overrides menu.
