The number one complaint I get is "I clicked 'release' on a quarantine email and it never showed up"
Is there a way to find out if they really did click release or if they accidentally clicked delete? and when it was logged so I can check the email server logs? I cannot find any way to get a log of what happened in the Fortimail.
I'm using a Fortimail 100 with 4.0MR3

yes, you can know it.
Look into AntiSpam logs, and search for strings like:
User quarantine messages released:user@domain:released 1 message(s)
under "Message" column
