You mean DNS name? If so then you need to setup a DNS server, then define the IP-name association in there.
If you mean on FortiGate for managing the IP/name in your firewall policies, then you need to go to menu: Policy & Objects > Addresses, then create Address with name Test1 and IP 192.168.0.4.
