Override username and password field' s missing

I upgraded from 4.0 MR2 patch 2 to 4.0 MR3. I didn' t like MR3' s lack of override options from the GUI, so I downgraded to 4.0 MR2 patch 6. Whenever I try to override a blocked web page now, I do not see a username and password field. Any suggestions or help would be appreciated. Thanks!

Best guess - missing " %%OVRD_FORM%%" from the replacement message. Attached is the default override form from v4 MR2 P6

I already had that code, thanks though. The solution I found was I had to change Override Scope from " User group" to " Ask" .
