Is there some trick to getting the OWA Agent to work?

I keep getting an web page that indicates that an application error has occurred.

I'm running the 4.3.2 VM.  OWA Agent 1.4.  Exchange 2010 on Windows 2008 R2 with .NET 4.5.2

The error in the Event Log has this Exception message: 

  Could not load file or assembly 'FAC_Agent.TwoFactorRedirection, Version=1.0.0.0, Culture=neutral,

  PublicKeyToken=4592dbf2287c19b8' or one of its dependencies.  The system cannot find the file specified.

I've tried running this on two different Exchange 2010 servers with the Client Access Role.

I tried moving the application to the ASP.NET 4.0 application pool.

I also tried moving the dll to the bin folder.

I also tried changing permissions on the folder to give Everyone read rights.

I also tried creating a different administrator account in the FortiAuthenticator.

I also tried installing several different Visual C++ runtimes.

I tried registering the DLLs, but they don't autoregister.

Nothing I've tried has worked.

I'm stumped!  Please help.
