FortiVoice 100D8 internal server error (web gui) & SSH connection refused

haved a FV 100D8 here on the bench. Device has been reset to factory settings (reset button until flashing red & release/reboot). 

Accessing device on LAN2 at 192.168.10.1 with Windows 10 machine plugged directly into port 2, with IP set static to 192.168.1.2, subnet 255.255.255.0, Chrome browser. We can get to the login page, and use the factory reset un/pw of "admin"/<blank> but we get an "internal server error" when logging in (after being redirected to 192.168.10.1/dologinaction.html). 

first login, we received the ssl certificate error message from Chrome (that the ssl certificate is invalid) but continued anyhow.

tried "SSH 192.168.10.1 -l admin -p 22" but all we get is that the connection is refused.

tried to telnet to box, but that doesn't work either... suggestions?
