Does the Enhanced Support Premium license for the FortiGate-100F include the provision of a replacement device if a malfunction requires repair at an external service center or replacement with a new unit while waiting for the repair or the new device?
Yes. It's called "Advanced Replacement" coming with Premium Support. See in the brochure below:

On a fresh install you wouldn’t have to worry about upgrade paths, they are in place to ensure config compatibility as you upgrade in service, fresh install you can just choose whatever version you want to run, install and then configure.
