There could too many reasons for this happening to list here.  Fortinet KnowledgeBase website has quite a bit of info on this. First Google results: https://kb.fortinet.com/kb/documentLink.do?externalID=FD33103 , https://kb.fortinet.com/kb/documentLink.do?externalID=FD40012

But debug is time consuming and then you need to know to interpret the results. Have you considered updating firmware to more up-to-date one? 5.4.1 is very, very old, so even if you open the ticket with Fortinet TAC the 1st thing they will advise is to upgrade to the supported FortiOS version, at least 6.0.12.

Thank you for your support.

I try to upgrade firmware to 6.0.12 but can not download it from support.fortinet.com

There is the message when I select download image firmware.

"Sorry, you don't have any product covered by Fortinet support contract. Please contact Fortinet partners to purchase Fortinet support contract or Fortinet customer service team at cs@fortinet.com"

Yes, you have to have a valid Support Contract assigned to your username in Fortinet website to be able to download firmware images. And if you try to keep the configuration, you have to upgrade in specific steps, so would need to download quite a lot of firmware images:
