Can anyone help with custom report on user activity

I'm trying to get a report showing users that access all blocked website categories

It needs to list blocked activity for every user we have

I need it to show in a table something like

time/date

username

web address

web category

blocked

so say jim goes on facebook and it is blocked it should show

29-09-16 @ 9:45AM - jim - www.facebook.com - social media - blocked

I've tried to use the inbuilt charts and datasets and also to write my own sql query dataset but I keep getting stuck, I can get some info but not in the format above.

I need to have this information for all users so I can schedule a report to be sent out at certain times, this is for the new school KCSIE regulations to do with e-safety.

Anybody good with writing queries like this?
