I have a chrome extension called unhook. Reduced YouTube to just a search bar at the top. No thumbnails, recommends, etc

Please check if you have "deep inspection" enabled in the firewall policy for "YouTube"- otherwise "blocking" just using web/application filter may not work correctly (because YouTube uses Google's *.wildcard security certificate).
Please refer below document for your reference.

Hello @HS08 ,
- Also make sure the URLs as mentioned in this article are also included in web-filter if you are using that: https://community.fortinet.com/t5/FortiGate/Technical-Tip-How-to-Block-Youtube-using-Static-URL-filt...

Please make sure to use proxy-inspection mode firewall policy in case you prefer not to implement deep packet inspection. Deep packet inspection require installing Fortigate CA certificate on every single PC that is coming from the incoming interface of that policy. If those aren't domain machines, this will be tedious. Proxy-inspection may work for you. This requires proxy-based UTM profiles.
