Description
This article describes an issue with Cisco WLC-5508 configured for FlexConnect which is not allowing Host Connections to SSIDs Requiring MAC Filtering. Log message received on the WLC-5508: Either Vlan Name id Template invalid or no name to id mapping exist for interface 'vlan_200 guest'

Solution
It was discovered that on a Cisco controller using FlexConnect, spaces are not allowed in the Interface Name.
Example:
WLC-5508 > Controller > Interface Name: vlan_200 guest
WLC-5508 > Wireless > FlexConnect VLAN > VLAN Template > VLAN Name: vlan_200_guest
To solve this error ensure that the Interface Name matches the VLAN Name.
