Solution
  1. Go to https://support.fortinet.com
  2. Select Support -> Manage Tickets.

  1. Select 'All Tickets' under Ticket.

  1. Find the ticket and select the ticket number.

  1. On the top right side of the screen and select Rate Our Service.

Alternatively, it is possible to select this link to go straight to the Ticket Survey option and then, type the Ticket Number.
