Make sure you have the old backup configuration file of the device. 
 

Step1:
++ Flash format the device.

Step2:
+ Formatting and loading FortiGate firmware image using TFTP:

Step3:
+ Once device is loaded with new image, refer below steps.:
+ Change the "config system admin" section and replace with old backup configuration of the firewall and add one more test user as below for safety purpose to login firewall.

edit "test"
set accprofile "super_admin"
set vdom "root"
set password test123
end

Step4:

+ And then load the old configuration backup file to firewall.
