How do I re-issue a fortitoken mobile after the user resets or uninstalls the app?

If a user's phone is replaced or upgraded -- or the end user uninstalled the fortitoken mobile -- what are the steps to re-issue the fortitoken mobile?

    Remove the token from the user, save, then add the token back to the user, save, then send activation code.

     

    I posted this today since I just had this issue.
