Fortigate + FortiAuthenticator with Wireless Self-Registration

We want use the Fortigate Wireless Captive portal with Self-Registration. The user will register him self on the Captive Portal to get the login for Wireless...

I've been configured the FortiAuthenticator with success. But, how is the HTML code that I need put on Fortigate captive portal to users access the self-registration portal?
