Troubleshooting Tip: Blinking LEDs or disk state failed on physical appliance

Description

 
This article describes how the following symptoms indicate a hardware failure has occurred:
 
LEDs are blinking amber on one of the drives, or:
 
Executing the command raid -ad -c=0  in the appliance CLI as root shows a failed disk state.
